Why the flared base is non-negotiable
The sphincter closes behind an object and draws it inward; without a stop wider than the opening there is no way back. A Swedish study published in 2018 in the International Journal of Colorectal Disease reviewed 85 patients who presented to emergency departments with a retained rectal object between 2009 and 2017, and the authors named the missing stopper as a recurring cause.
Which size to start with
Start smaller than you think. For a first plug a maximum width of around 25–30 mm is sensible — comparable to a finger — and every step up should be small and unhurried. Metal and glass plugs are heavier and hold temperature, silicone is softer and more forgiving, and plugs with a tail or vibration are an upgrade, not a starting point.
Lubricant is not optional
The rectum does not lubricate itself, so lubricant is a condition, not an accessory. The WHO/UNFPA advisory note reports a prospective study in which the condom breakage rate during anal sex was 3% with water-based lubricant and 21.4% with none — the same physics applies to toys. For rectal use WHO gives a target pH of 5.5 to 7, because the rectum is close to neutral, and recommends lubricants below 1200 mOsm/kg.
